Summer Camp 2023 & School Year 2023-2024
Registration Instructions for New Students (Please only complete this process if you have been offered a spot in the program).
Part 1. Please complete, print and sign the following form. DSS does not accept electronic signatures so an actual signature is required. Every line must be completed, so please write N/A if it does not apply. If you would then like to upload the form, you may upload it to Brightwheel or you may submit the paper copy with your payment.
DSS Form 2900 - Please download, complete, save and print. This form is required every year.
Part 2. Please complete and submit the following electronic forms.

Rules, Seatbelt Agreement, Three Strikes Policy & Electronic Policy Acknowledgement
Pick-up Authorization Form - Only complete this form once per family. The address, phone number and driver's license numbers are required for all people that are authorized to pick-up.
Part 3. Please provide the DHEC S.C. Certificate of Immunization from your doctor. You may upload it to Brightwheel here or you may submit the paper copy with your payment.

Part 4. Please turn in the signed DSS Form 2900 and DHEC Certificate of Immunization (upload or turn in the paper copy) along with your registration fee, summer activity fee and summer refreshment fee (if applicable) at the time of registration.
