DHEC S.C. Certificate of Immunization Form

Pick-up Authorization Form - Only complete this form once per family.
Elementary Permission & Signature Page - Only complete this form once per family.
Pandemic Sick Policies and Release Form - Only complete this form once per family.
Swimming Ability & Sunscreen Form - Only complete this form once per family.
