Please complete and print the following forms:
DSS Form 2900 - Please download, complete, save and print. This form is required every year.
Please complete and submit the following electronic forms.
Forms are completed on a separate, secure website so you
will need to return to this website to complete each subsequent form.
Elementary Program Registration Form
Pick-up Authorization Form - Only complete this form once per family.
Elementary Permission & Signature Page - Only complete this form once per family.
Pandemic Sick Policies and Release Form - Only complete this form once per family.
Swimming Ability & Sunscreen Form
Please provide the following form from your doctor.
DHEC S.C. Certificate of Immunization Form
Please turn in the printed forms and immunization form, along with your registration
fee, summer activity fee and summer refreshment fee (if applicable) at the time of registration.
